Abstract

Experimental evidence suggests that altered muscular function can influence craniofacial morphology. The switch from a nasal to an oronasal breathing pattern induces functional adaptations that include an increase in total anterior face height and vertical development of the lower anterior face. While some animal studies have suggested predictable growth patterns may occur, studies in human subjects have been much more controversial. Therefore, individual variations in response should be expected from the alteration of a long face syndrome patient's breathing mode.

Abstract

Longitudinal growth changes in the soft tissue profile of 40 Caucasians between the ages of 7 and 18 were studied. The sample consisted of 17 males and 23 females who had Class I dentitions and balanced faces at age 7 and 18 years. None of the subjects received any orthodontic treatment. Cephalometric radiographs were available, as a rule, on a yearly basis.

Soft tissue thickness, measured at the nose, upper lip, lower lip and chin, as well as the length of the upper and lower lip, all increased by varying amounts over the period of the study. Females acquired more growth as a percentage of their adult size (at age 18) than males in all variables except the angle of inclination of the skeletal chin which increased more in males. The largest increase in relative size was noted in the nose measurements. In males, the nose had not attained adult size even at age 18. Upper lip length growth, on the other hand, in both males and females was complete by the 15th year. The difference between male and female lip length growth was clinically significant; the average aggregate increase in upper and lower lips combined in males was 6.9 millimeters compared to 2.65 millimeters in females. The total gain in thickness at laberale superius was over four times as much in males as in females and continued to increase in males even at age 18. The change in thickness of the soft tissue at pogonion was not large, an average of 2.4 millimeters in males and 1.5 millimeters in females. The major contribution to the anterior growth of the chin was translatory, contributed largely by growth in the skeletal length from pogonion to pterygomaxillary plane.

Abstract

Cephalometric radiographs from a sample of 64 untreated persons (32 Class I and 32 Class II) were evaluated to determine the amount, direction and timing of facial soft tissue development. Twenty-five parameters were evaluated in the mixed dentition (7 to 9 years), the early permanent dentition (11 to 13 years), and early adulthood (16 to 18 years). Results showed that anteroposterior growth and subsequent increased anterior projection of the nose continued in both males and females after skeletal growth had subsided. However, females had concluded a large proportion of their soft tissue development by age 12 while in males continued growth was noted until age 17 resulting in their having greater soft tissue dimensions for many of the parameters evaluated. During the developmental period, the angular shapes and positional relationships of the nose, lips and chin remained relatively constant for both sexes and was relatively independent of the underlying hard tissues. Treatment planning implications may be drawn from the amounts and timing of the soft tissue development found in this study.

Abstract

Two studies related to soft tissue profile change, are reported in this paper.

In the first study, 50 adults with normal occlusion were compared to 50 adults with maxillary protrusion. Cephalometric radiographs were obtained for all subjects in two positions: with lips relaxed and lips closed. The amount of lower lip movement increased in relation to the degree of maxillary protrusion. In both groups, thickness of soft tissue pogonion decreased with upward movement of the lower lip.

In the second study, 20 posttreatment patients were examined to determine how soft tissue profile is related to the morphologic alterations of treatment. Ten patients were treated for maxillary protrusion and 10 for bimaxillary protrusion. Upper lip retraction was about 40 percent of the maxillary incisor retraction. Cases with maxillary protrusion showed slightly less soft tissue change than cases with bimaxillary protrusion. Lower lip retraction was about 70 percent of the maxillary incisor retraction.

With an understanding of pretreatment lip posture and data related to the soft tissue response to treatment, a framework is presented for predicting a patient's posttreatment facial profile.

Abstract

The purpose of this study was to explore the possible significant sex difference in occlusion, provide information about the occlusal variation among Egyptians, and present an epidemiologic panorama of dental occlusion among different ethnic world populations. The hypothesis was that the occlusal variation is not independent of sex.

A sample of 501 female and male adult subjects was studied. Normal occlusion, Angle's classification of malocclusion, and the Dewey-Anderson modifications for typifications were recorded. Chi-square tests were used.

The results obtained from this study indicate that a significant sex difference in occlusion exists for normal occlusion, Angle Class I, and Angle Class III. Further, considering an anterior crossbite as the sole indicator of an Angle Class III malocclusion is erroneous; an anterior crossbite may exist in other classes, and Angle Class III type 1 (edge-to-edge) is more prevalent than either Class III type 2 (normal anterior overbite) or type 3 (anterior crossbite). Although numerically different, occlusal variation follows a universal general distributional pattern for most world populations. Some speculations are presented for clinical implications and for research suggestions.

Abstract

The tonic(resting) and peak (maximum) lip pressures of 100 subjects were measured. The purpose was to evaluate the validity of the assumption that high peak pressures are indicative of high tonic pressures and vice versa. This paper describes the equipment and techniques devised to test these pressures. The results showed no statistically significant relationship whatsoever between tonic and peak lip pressures. We, therefore, suggest that maximum lip pressures not be used as a diagnostic aid in orthodontics.

Abstract

The morphology of the face and dentition of 62 adult professional wind instrument musicians was recorded with roentgencepha-lometry and dental casts. The musician group was comprised of 31 brass instrument players, and 31 reed instrument or flute players. The results were compared between the two subgroups and between these and a control group. The few differences found between groups were small. Overbite and upper dental arch width at the canines were smaller in the musicians than in the control group. Width at the maxillary and mandibular molars was somewhat smaller in the reed instrument and flute players than in the control group. No difference in overjet was found. Cephalometric analysis showed similar, normal facial morphology in all groups. The range of variables recorded was large in all groups.

Abstract

There is growing consensus that upper airway obstruction is a causative factor in nocturnal enuresis. This phenomenon has an unhappy history, although some surgeons in the past have touched on its treatment through the relief of upper airway obstruction. Only recently have sleep laboratory investigations presented a clearer, though still incomplete, picture of the etiology of nocturnal enuresis through disturbed sleep patterns. The obstruction is usually an adenoidal hypertrophy or, less commonly, an anterior nasal stenosis. While the otolaryngologist can readily cope with the former, surgical difficulties make treating the latter problematic. In many cases, the constriction can be reduced by rapid maxillary expansion. In the ten cases examined in this study, nocturnal enuresis ceased within a few months of maxillary expansion.
